This item can only be ordered through Purchasing on a web requisition. Documentation and control issues require tracking of the orders and that licensing and purchase documentation be maintained.

Definition: Controlled substances are drugs or other substances, both narcotic and non-narcotics, that come under the jurisdiction of the DEA Controlled Substances Act, and are listed in the DEA Controlled Substances Inventory List.

Procurement Procedure . . .

» Submit a web requisition to Kelly Freel Email: Kelly Freel (kmfreel@iastate.edu) with the DEA license number on the face of the requisition.

For class I and II substances, we will require a DEA 222 form be attached to your requisition.
